The attitude was off the charts odd.

People yelled. People refused to wear masks. People (non-patients) were hauled out of emergency rooms by security. People accused healthcare of lying about diagnoses. People accused healthcare of being paid off to lie about all covid diagnoses (by whom is far less clear). People went so far as to tear breathing apparatuses off the “loved” one it was helping breathe. People won the country a full hospital ban on visitors that didn’t fully go away until very recently. People died of covid or covid complications and their own people still didn’t believe Covid existed.

And why? Because they didn’t want to wear a mask? Because they didn’t want to stay home from work when sick? (Remember, everyone mostly dosed themselves with cold medicine and then took their crud into work, infecting everyone, in before times.)

There’s The Money, and then there’s everyone else.

At this point though, they’re even taking art away from us. They’re not making anything though, only manufacturing trash into giant piles.

End game? No one knows. Not even them.

Once again I’m reminded of Season 1 of Black Mirror. Second episode? Worker drone is “home”, “relaxing” in his sleep cubby and the walls are a barrage of ads. Occasionally he taps to pay credits to shut one down as the rest continue, as casual as anyone scrolling in boredom.

Or a shut down golden retriever in those old, now illegal learned helplessness experiments.

We’re fully Linux now, various distros per machine, so I no longer pay much attention to windows news.

I was at my parent’s house, trying to add some items to the smart TV. They started listening to these awful AI interpersonal dramas read by AI, on YouTube.

My thought was to add some podcasts and their Audible account, for more selections. Maybe they’ll still only listen to that, even so. You can’t add much to a smart tv. No podcasts. No podcast app. A pretty spartan selection of apps. Even something as large and mainstream as Audible. No go. Libby? Yeah, right.

I don’t get it. What’s the point? Just buy a monitor, jack it into a PC for media, and have everything you want minus this telemetry.

I don’t lie, I’ve never been good at it, but I don’t elaborate. If I’m asked something I don’t want to answer I say I’m not discussing that here.

There’s a 98% respected rule to not discuss politics. Management states they’d prefer we don’t because it upsets the clientele and it’s irrelevant to the workplace.

However, every couple years there’s that one person who cannot read the room and loudly talks politics to the quiet people around her she thinks are listening but who are instead cringing as they wait for her to stop talking.

They are testing quite a few things atm. Radiology image recognition is definitely there. But is this what the phone wavers are accessing? No. The Google search bot and the bots being tested are two different things.

I wouldn’t get too excited about the future of AI in medicine, not given the level of venture capital and such in medicine already.

LLMs are programs that can be tuned by people who have not sworn an oath or spent 12 dedicated years and $250k debt to pursue medicine. What will motivate how these bots are tuned? Publicity and PR can create excitement, but what have the last 30years of medicine taught us? Even the radiology bot can be “turned down” in ways a human rarely would allow.

Profit speaks. Profit drives. Profit decides patient care levels and outcomes. What keeps it afloat are the people clawing back at administrators. Don’t underestimate the power of angry nurses or pushy physicians in just how much they’ve kept healthcare afloat. For you. While admin battles them from the shadows in the name of profit.

What they don’t mention among the many bots being tested is the EOLI, or, End Of Life Index. The most used electronic medical record had a bot introduced this year, unannounced, that is a probability predictor of a patient dying in the next 6 month. The mouse over admonishes caregivers not to discuss this with patients or families as it may upset them. Does/will the EOLI factor into insurance decision making?
