It’s much more likely to be indicative of a mental health issue, even if it’s just anxiety. Insecurity around how one looks it’s usually related to unrealistic societal pressures and bullshit public “standards” as to what qualifies as beautiful.
This usually occurs to women much worse than men and is often directly tied to their “value”.
It’s easy to see someone fuck their shit up with plastic surgery and go “haha, that’s your choice”, but, in general, our modern western society is hyper individualistic, where people’s difficulties are said to be personal failings. Whereas as a socially evolved species, it’s much more tied to our peers and culture than people either realise, or want to admit.
Plus it’s easier to ignore these things when you don’t like the person at the center of the current discussion
