Yep, they are considered separate holes. So for example. “Mouth to the ass” is one hole and the two nostrils add two holes to the total.

I don’t think it really matters how big the hole is so as long as the opening isn’t fused maybe it still counts as a hole. But really, I think whenever your mouth is open or not is out of scope for topology.
